Keloids Treatment - How to Treat Keloid Scars

Each process has its particular length of time and results may vary from being short term to permanent. No treatments are guaranteed to cure or prevent keloid scars but in many cases proper treatment may reduce the abnormal growths left on the skin, making them less noticeable.

It is believed that there is very little that can be done to prevent keloid scars from forming. However you can assist the skin by keeping the wounds clean and well dressed. There is no single treatment for keloids, and most treatments do not give completely satisfying results. Two or more treatments may be combined. If you decide to pursue treatment for a keloid scar, you will have the best results if you start treatment soon after the keloid appears.

Available keloid treatments include:

Removal with conventional surgery: This is a fairly unreliable technique that requires great care. However it still remains one of the most effective and of the least complex of all treatments. The recurrence rate is thought to be about 50%. The keloids scars generally return after being removed and in some cases may appear larger than the original. The scars are less likely to return if surgical removal is combined with other treatments.

Dressing: Moist wound coverings made of silicone gel sheets have been shown in some studies to reduce the size of keloids over time. This treatment is safe and painless but difficult to comply with and reports from many medical sources say their efficacy is questionable.

Corticosteroid injection: Injections with triamcinolone acetonide or another corticosteroid medicine are typically repeated at intervals of four to six weeks. This treatment may reduce keloid scar sizing and irritation, but the injections are uncomfortable. Another option is to combine surgical removal with these steroid injections- usually applied one at the time of the surgery and then later after about 3 to 4 weeks. This treatment has been reported to have between a 50% to 70% rate of appearing again in latter years.

Compression: This involves using a bandage or tape to apply continuous pressure 24 hours a day for a period of six to 12 months. Compression can cause a keloid to become smaller. For keloids that form at the site of an ear piercing, a clip known as a Zimmer splint usually reduces keloid size by at least 50% after one year of compression. Zimmer splints that look like earrings are available. Prolonged compression of scar tissue can theoretically soften and break up keloid scars, however the practicality of this option depends on the location of the keloid.

Cryosurgery: This freezing treatment with liquid nitrogen is repeated every 20 to 30 days. It can cause a side effect of lightening the skin color, which limits this treatment's usefulness.

Radiation therapy: Another option combines surgery with external type radiotherapy. This therapy is controversial because radiation increases the risk of cancer. Radiation treatments may reduce scar formation if they are used soon after a surgery, during the time a surgical wound is healing . Radiation has the effect of interfering with skin growth (fibroblasts) and collagen production. Research varies on which type of combination therapy is the more effective. Both radiotherapy and steroid drugs have side effects so you need to discuss with your doctor the most effective. It may be worth getting a second opinion before proceeding with either treatment.

Laser therapy: This is an alternative to conventional surgery for keloid removal. There is no good evidence that keloids are less likely to recur after laser therapy than after regular surgery.

Experimental treatments: One treatment showing promise is injecting keloid scars with medicines that were developed to treat autoimmune illnesses or cancers. Treatments with these medicines (various types of interferon and the chemotherapy agents 5-fluorouracil and bleomycin) will need to be evaluated further before they are appropriate for use outside of research studies.

Other non-surgical interventions that are currently being tried with varying results include antihistamines, vitamins, nitrogen mustard, Verapamil, retinoic acids. Interferon therapy (drugs acting on the immune system), has been reported as effective in reducing keloid scarring however it does have significant side effects. Examples are toxicity, flu like symptoms, depression, nausea and vomiting.
