What is actinic keratosis?

It is a scaly or crusty bump that arises on the skin surface. The base may be light or dark, tan, pink, red, or a combination of these, or the same color as your skin. The scale or crust is horny, dry, and rough, and is often recognized by touch rather than sight. Occasionally it itches or produces a pricking or tender sensation. It can also become inflammed and surrounded by redness. In rare instances, actinic keratoses can even bleed.

The skin abnormality or lesion develops slowly and generally reaches a size from an eighth to a quarter of an inch. Early on, it may disappear only to reappear later.

Actinic keratosis is most likely to appear on the face, ears, scalp, neck, backs of the hands and forearms, shoulders, and lips - the parts of the body most often exposed to sunlight.

Actinic keratosis is the most common sun-related growth. An estimated 60% of individuals older than 40 years who are predisposed have at least one actinic keratosis or solar keratosis. Usually, these people are fair-skinned, burn easily, and tan poorly, as well as have occupations or hobbies that result in excessive sun exposure. They may be treated effectively by available methods. Actinic keratoses have the potential to develop into skin cancers; therefore, treatment is indicated.

Causes: Increased sun exposure and higher intensity exposure increases the incidence of actinic keratoses. Lesions flare and become more visible during periods of immune suppression, such as acute sun exposure or chemotherapy, especially with systemic 5-fluorouracil. However, the development of actinic keratoses would not occur without sun exposure.

Medical management begins with educating the patient to limit sun exposure. Patients should be cautioned to avoid sun exposure from 8:00 am to 4:00 pm as much as possible. They also must wear adequate sunscreens and protective clothing daily.

Prevention:

• Actinic keratoses can be reduced or delayed by using sunscreens and reducing sun exposure.
• Patients should limit recreational exposure and those who work outdoors should consider making adjustments in their work-related sun exposure.
• For patients forced to undergo sun exposure, recommend applying a sunscreen of sun protection factor (SPF) 30 or more and wearing protective clothing daily.
